EXCHANGE POLICY
Only the defective unit will be warranted. Accessories (power cable, shrouds etc.) will not be exchanged unless it’s determined to be manufacturer’s defect.
If new accessories are required with warranty replacement unit, place an ( X ) in parts box above and indicate on the unit. A parts fee will be applied and the
amount will appear next to parts request box when the return authorization number is issued and faxed back.

SHIPPING GUIDELINES:
1. All section of RA form must be completed and faxed back in order to receive an RA #. DO NOT SHIP WITHOUT RA#
2. RA # must be written on outside of all returned boxes and a copy of the completed RA form must be enclosed.
3. Customer is responsible for shipping and insuring the product being returned to indicated mailing address above.
